The USPSTF recommendations have been beneficial in generating a rigorous dialogue regarding the advantages/disadvantages of prostate screening. Their recommendation of D could cause CMS to stop covering the cost of the test, which will have very serious implications for many high risk patient populations. The PSA is not a perfect screening test, nor am I aware of any other perfect screening test for prostate cancer or any other of the more common and lethal malignancies; hence, the PSA must be use judiciously by informed clinicians (primary care and urology) in order to allow for the most appropriate identification of patients who are at risk of suffering from clinically significant prostate cancer, the 2nd leading cause of male cancer death in the United States.
