The decision to start chemotherapy is complex, and is becoming more complicated with the approval of new hormonal agents, as well as the likely approval of new radiopharaceuticals. Chemotherapy is indicated for patients with castration resistant symptomatic metastatic disease. Whether it should be employed earlier in the course of disease is controversial, but I consider it in patients with castration resistant prostate cancer who have rapidly progressive disease, or visceral sites of metastases.
