In what cases is hormone therapy recommended to treat prostate cancer?

There are 3 indications for hormone therapy for which we have randomized trial evidence supporting its use: 1) patients with intermediate/high risk localized prostate cancer patients undergoing external beam radiation therapy to the prostate 2) patients who have positive lymph nodes post radical prostatectomy and 3) patients with metastatic disease. The most common use of hormone therapy is for those asymptomatic patients with a rising PSA and no evidence of metastatic disease. Unfortunately, there are no randomized trials of early vs delayed androgen blockade in these patients to support or refute the earlier use of androgen blockade in this state of disease.
